PSQH: The Podcast Episode 43 – The Value of Transitional Care Management

On episode 43 of PSQH: The Podcast, Mike Case Haub, CEO of CHC Health, talks about transitional care management.

Mike Case Haub, PharmD believes that the future of healthcare will be a patient centric, team-based approach where coordination of care and transparency will ensure that patients, as consumers of healthcare, are able to make informed decisions regarding their healthcare options. Mike’s goal for CHC Health is to positively impact outcomes through a pharmacist led approach engaging patients, caregivers, prescribers, and other healthcare professionals. He believes that by removing current barriers, pharmacists can practice at the top of their license. He also believes in the concept of “unbundling pharmacists from pharmacy” where a pharmacist role is not primarily a distribution of a product, but a medication expert focused on the management of medications.

Mike Case Haub has worked in pharmacy for almost 30 years starting with an independent pharmacy in Osceola, IA as a teenager. He has worked in numerous different types of pharmacy through the years: independent, chain, hospital, investigation research, government and managed care. In each of these practice settings Mike always tries to focus on pushing the profession of pharmacy to the next level of practice through innovation and advocacy.

Prior to founding CHC Health, Mike was the Director of Network Performance at OutcomesMTM, a Cardinal Health company, where he collaborated with executive management at health plans, pharmacy entities and technology vendors to operationalize medication management services. Mike was also the subject matter expert on system integrations and interoperability.
